My mother lived at home alone into her 98th year.  She  recognized
a myocardial infarction and called the hospital literally next door  to
come pick her up.  Medicare, for some reason, refused to pay
initially, and the ambulance charge (1998) was more than $500
for transporting mother about 100 feet to the hospital ER.
 
Eventually Medicare did cover this, but there was a reduced fee.
