Amen.  Actually, on a lot of these.  But I don't think so far anybody 
has mentioned "would of", "should of", or "had of".  I think those came 
from people saying "would've" as a contraction for "would have", etc., 
and kids listening heard it as "would of" and incorporated it into their 
own speech (where it didn't "show") and writing, where it did.  Then the 
"had of" came from that "of" usage directly, since "had" is already a 
form of "have".  Maybe.  But the "of" has no place in a lot of sentences 
where I see it.
