Possibly *ultimate* wrong reading faux pas - my son accidentally turning page and reading gospel at mom's funeral, instead of the NT lesson. Priest quietly freaked, and the deacon (who was to have read gospel) thanked Verni's grandson for reading gospel and then he read NT lesson ( in RC church)

The rector started his sermon by saying, "If you're having trouble
following the service today, you're not alone...Here's the reading
from Acts." I apologized after Mass and he said, "Oh, no problem!" But
I knew for him it was probably a huge problem...I'm sure he swore
about it at some point to someone.

If it was something I was using, I found a way to read the parts I needed into the sermon. Most parishes here have the readings in their bulletins, so they would notice the mistake anyway.
