I presume you have an end-of-life arrangement made.
 
Through the years I saw many ALS patients because some of them
have severe seborrhea - oily skin with red facial and scalp scaling.
Sometimes the seborrhea is intractable.
 
One patient stands out.   He was a PhD in Bacteriology, and  suddenly
went downhill with his ALS.
 
He lived about 15 years on a ventilator at the VA, his wife resolutely 
refusing to pull the plugs.  For the last 5 years of this period he  could
move only his eyelids.  As I recall, one blink was "yes" and two  blinks
were "no".  
 
He finally, and mercifully died.
 
It was not the course that most of us would have chosen for him
