I have no experience with such late and life crises, but I do at the
senior in medical school level.  No. 2 in standing classmate had  a
problem with a pulmonary specialist, and after one encounter  with
her (she was a classmate of my older brother, Jim - Class of 1946)
and he just gave a flip answer, and walked out of the Medical Center.
 
He returned to finish up the following year, so he was a year late.
