Not necessarily so.  In this country at least, and I think probably in 
the USA as well, someone has to live as the new gender for a while 
before surgery, and probably also hormone treatment start.  At that 
stage the signs of the old gender are obvious if you look.  For example 
I recall noticing someone's adam's apple as I was serving them at the 
Beer Festival a few years back.  Since then their look (I have seen them 
each year since) she has become more feminine-looking.  I have also seen 
someone else in this small twon whose male looks are quite obvious 
despite the female garb.
