I had a fancy script version of it on the cover of my mother's funeral
program in 1998.  Everything needed for that liturgy was in that 
20 page funeral program which was a blending of Methodist,
Episcopal, Lutheran, and Roman Catholic material the likes of which
was unique in that Methodist setting.  This all played tribute to the
various branches of the family, and included a Eucharist, the latter
using the John Wesley text which is essentially the Cranmerian
Anglican text.
